Child Plan (0-13)

Child plan is suitable for children age 13 and younger and includes the following annual services:

  • 3 Cleanings

  • 3 Fluoride Treatments

  • 2 Doctor Exams

  • 1 Annual BWX radiograph

  • Oral Cancer Screening

  • Oral Hygiene instructions

  • 1 Emergency exam includes 1 radiograph

  • FMX/Panoramic radiograph every 3 years

Adult Plan (14+)

For our adult patients with healthy teeth and no periodontal disease, we offer a plan that provides the following annual servies:

  • 3 Cleanings

  • 3 fluoride treatments

  • 2 Doctor Exams

  • 1 Annual BWX radiograph

  • Oral Cancer Screening

  • Oral Hygiene instructions

  • 1 emergency exam including 1 radiograph

  • FMX/Panoramic radiograph every 3 years

Perio Plan (14+)

Our Perio Plan is specifically designed for our patients with symptoms of periodontal disease and provides the following annual services:

  • 4 Cleanings

  • 4 Fluoride Treatments

  • 2 Doctor Exams

  • 1 Annual BWX radiograph

  • Oral Cancer Screening

  • Oral Hygiene instructions

  • 1 Emergency exam including 1 radiograph

  • FMX/Panoramic radiograph every 3 years

Terms and Conditions

Membership is effective on the date in which payment is received and terminated one year from the initiation of membership. Payments are due the day services are rendered. Accepted payments forms for service fees are cash, check and credit card. Membership benefits include discounted dental services at 25% off our original and updated fees as of 11/15/23. This is not an insurance plan and can only be used at Northwoods Family Dental. The plan is not transferrable and cannot be used with any other dental coverage. It is the sole responsibility of the member to maximize their benefits by arranging all of the appropriate appointments within the 12 month membership term. Whitening products, tooth brushes, MI paste and other dental products are not included and there are no cost reductions for these products. Northwoods Family Dental is not liable for any services treated outside of the dental office with any dental or medical specialists. Our office fees are subject to change at any time. All appointments must be cancelled or rescheduled with a 48-hour notice to avoid cancellation fees. Patients may be dismissed from the program if not abiding by our payment or cancellation policy. There are no refunds for membership programs purchased or unused benefits. *Discount benefits are not eligible towards botox*
